I agree to release, forever discharge, & hold harmless “Sacred Soul Tattoo” & its agents/employees off from any & all claims or legal actions arising from or connected in anyway with my tattoo/piercing or the procedure used during my tattoo/piercing*

This includes any damage to clothing or personal belongings that may be damaged with any products used related to my tattoo/piercing.*

I acknowledge that by signing this waiver I have been given full opportunity to ask any & all questions which I might have about obtaining my tattoo from Sacred Soul Tattoo, & that all my questions have been answered to my full & total satisfaction.*

I acknowledge that I am not pregnant/breastfeeding*

I acknowledge I am not under the influence of drugs or alcohol.*

I acknowledge I am free of communicable diseases.*

I acknowledge that I have truthfully represented to the employees, agents, and representatives that I am over the age of 18.*

I acknowledge that infection is possible result of obtaining a tattoo/piercing, particularly in the event that I do not properly care for my tattoo/piercing.*

I acknowledge that obtaining a tattoo/piercing is my choice alone and consent to the application of a tattoo/piercing and to allow any reasonable actions or conduct by the employee necessary to the application of my tattoo/piercing.*

I acknowledge that it is not reasonably possible for the representatives and employees of Sacred Soul Tattoo to determine whether I might have an allergic reaction to the dyes, pigments, or process used in my tattoo. I agree to accept the risk.*

I acknowledge that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body.*

I acknowledge that a tattoo is a permanent change to my body and that no representations have me made to me as the ability to remove or change my tattoo later.*

I acknowledge that receipt of verbal and/or printed instructions advising me on proper care of my tattoo/piercing and I recognize the absolute necessity of following those instructions.*

Signing this waiver I am giving up all rights to take legal action, charge, or file complaints against this tattoo studio & employees from any claims/damages from or connected to in any way with my tattoo/piercing or procedure current in the future.*

I consent to giving Sacred Soul Tattoo the rights to publish any & all photos of my tattoo, piercings, toothgems.*
